* Medical doctors statement, certifying a standard of health acceptable to Centennial College, clinical and affiliated EMS agencies. The statement must be as outlined in the Ambulance Act, used in practical experience, and as required for a Class F drivers license

o Received six or more demerit points on his/ her driving record
o Had his/ her drivers licence suspended in the previous two years
o Been prohibited from driving under the Criminal Code of Canada within the past three years
o OR has been convicted of any crime involving moral turpitude for which she/ he has not been pardoned
* B) Requires an Ontario Secondary School Diploma or equivalent.
